act on Platinum (GSTP1, XPD, XRCC1)
Patients with wild type GSTP1 suffer from severe neuropathy, while patients with heterozygous mutants or homozygous mutant show less neuropathy and have a better response, and longer survival from Oxaliplatin. Mutations in XRCC1 and XPD gene are associated with decreased overall survival in patients taking cisplatin.

Homozygous mutants in UGT1A1 gene are at a high risk of diarrhea and neutopenia with Irinotecan, while patients with wild type or heterozygous mutants are at a low risk.

Patients with mutations in CYP2D6 are poor metabolizers and are at a higher risk of relapse as compared to wild type patients. These mutants should use aromatase inihibitor in place of Tamoxifen

Patients having mutations in this gene cannot tolerate standard dosage of 6-MP. Population studies show that, 10% patients are heterozygous mutants and 0.3% are homozygous mutants and they should get 50% dose and 5% dose respectively. Patients with mutations in TPMT gene show toxicity from Mercaptopurine.

Mutations in MTHFR gene is directly related to toxicity from Methotrexate. Dose reduction is warranted in homozygous mutants and lower starting dose is recommended for heterozygous patients. Patients with mutation in RFC1 show toxicity from Methotrexate.
